Re-Enrollment Application for Undergraduate Students

Former students wishing to return to ÃÛÌÒÓ°Ïñ to complete additional course work must submit a Re-Enrollment Application to the Registrar’s Office.

1. REQUEST: Please complete the Application for Re-Enrollment (PDF). Type your responses into the fields or use the mouse to select options, then print and sign the form. IMPORTANT: To comply with federal regulations governing the privacy of educational records, ÃÛÌÒÓ°Ïñ requires the penned signature of the student to release academic records. Electronic/digital signatures will not be accepted.

2. UPLOAD: Scan your signed/completed form and upload it using ÃÛÌÒÓ°Ïñ's Secure File Upload Service. to upload your request.

You will receive a notification once your file has been uploaded successfully, as well as an email confirmation to the address you provided. Once completed, please submit official transcripts of any post-secondary coursework since terminating your studies at ÃÛÌÒÓ°Ïñ to the Registrar’s Office. No application fee is required.
